The one that has paid me the best is Opinion Outpost, payout is $5. They send point surveys, 10 = $1.00, 20 = $2.00, etc. I have cashed out 3x. My last cashout was for $15.00, (requested 06.30.09 and received the check (no fees) by 07.04.09. They are located in Utah (USA). They also pay you $1 per referral, after they complete their first survey. I just cashed out there 06.30.09 and my balance is already back up to $9.50 (surveys are nearly all 20 & 30 points).

Global Test Market, do pay had 2 cheques from them However they have slowed down on surveys in the last month. Payment by cheque.
