Im not sure about USA only, but there are a few that are tier 1 only....Cash Desks and Adpaid in particular comne to mind

There are a few sites that accept members from USA only.

Don't think that there is any PTR site that only accepts members from Canada... Maybe someone who actually knows instead of just thinks might venture to answer that...
